participating provider for a reduced benefit.
- Coordinated Care – members
choose a Personal Care Provider (PCP) who coordinates
referrals to in-network specialists.
- Self-Referral Options –
members do not need a PCP referral to access providers
for covered women’s health-care services, smoking
cessation, routine vision and hearing exams, and covered
chiropractic services.
