| Regence Disease Management offers early intervention and long-term management of chronic conditions. This can help reduce the need for expensive procedures, hospitalizations and emergency room visits. Our internal programs focus on the conditions with the most impact on our members’ quality of life.
- Asthma. One of the most common diseases in the United States, it is becoming increasingly prevalent in both adults and children.
- Chronic Obstructive Pulmonary Disease (COPD). A chronic lung disease that includes two main illnesses: chronic bronchitis and emphysema.
- Diabetes. Associated with an increased risk for many serious, and sometimes life-threatening, complications.
- Congestive heart failure (CHF). A leading cause of hospitalization in certain age groups.
- Coronary artery disease (CAD). The leading cause of death and premature permanent disability for men and women in the U.S.
- Anxiety. Anxiety disorders affect up to 17% of the population and often complicate many medical conditions.
- Depression. As many as one in ten people may experience depression at some time in their lives.
How the programs work
Health promotion nurses and behavioral health care coordinators work one-one-one with our members:
- Members receive tailored interventions, including care reminders, newsletters, educational materials, clinical support and other outreach as needed, based on unique individual needs.
- Education helps members understand the care plan their physician recommends, and how to stick to it.
- Seamless coordination with other programs as needed, such as case management and Special Beginnings.
