Employee Level Template: Instructions
For each row in the spreadsheet, an employee record will be added to the quote.

Employee & Dependents Template: Instructions
For each row with a value of "Subscriber" in the relationship column a employee record will be added to the quote. For each row with a value of "Spouse" or "Child" in the relationship column, dependent records will be added to the preceding employee record.

Custom Format (Employee Level): Instructions
For each row in the spreadsheet, an employee record will be added to the quote. If you already have census data in a spreadsheet with similar columns, but the column names are different and/or are arranged in different order, use this approach. You will be prompted to map the columns appropriately.
