09/29/2011   January 2012 Group Master Application (GMA) forms now available

Group Master Application (GMA) forms (1 to 99 and 100+) for Jan. 1, 2012 effective dates have been updated and will be posted to the Forms section of our Agent website, as well as to our Employer website and the Agent Toolkit. ASC GMAs are available through your Regence Sales contact.

Please note that our pre-filled GMAs available in Agent Center for renewing groups haven't been updated to reflect these changes. We'll notify you when those forms have been updated.

Fillable GMAs will also be available shortly; we will post those when they are ready.

Changes that have been made to these forms include new benefit options, such as our new offering Regence Employee ChoiceSM. To assist you with these changes, we created a Guide to Changes for each updated GMA.

As a reminder, groups renewing without any benefit plan, contribution or participation-level changes don't need to complete a GMA.
