July submission deadlines approaching for new groups
All required forms must be complete and submitted to us by the published deadlines to guarantee effective dates and rates; no exceptions. Incomplete or inaccurate information will delay the effective date of group coverage.
The following information and forms are required:
- Group Master Application
- Application for Enrollment/Change or Enrollment Spreadsheet
- Waiver Form
- Affidavit of Qualifying Domestic Partnership (if applicable)
- Affidavit of Qualifying Incapacitated Dependent Eligibility (if applicable)
For groups with Aug. 1, 2011, effective dates, enrollment information will be due on the following dates:
- July 20, 2011: Paper Enrollment Applications are due by 5 p.m. (Pacific time).
- July 25, 2011: Online or Spreadsheet Enrollment Information is due by 5 p.m. (Pacific time).
To be consistent with these deadlines, Agent Center will close for quoting at 5 p.m. (Pacific time) on July 20, 2011, for quotes for coverage effective Aug. 1, 2011.
Recently we announced that micro (1-4 subscribers) groups will have a different submission deadline than the 5-99 groups. Starting with effective dates of Sept. 1, 2011, all new micro groups must be submitted by the 15th of the month immediately preceding the effective date (e.g., Aug. 15 for a Sept. 1 effective date). All necessary documents, as detailed in the Micro Group reference guide, must be submitted on or before that date. If additional information is required, we need to receive it by the 20th of the month immediately preceding the effective date; otherwise, we'll move the proposed effective date out an additional month.
