The Standard Health Questionnaire (SHQ) exceptions page has been revised
The exceptions page (page 2) of the Standard Health Questionnaire (SHQ) for Washington State has been revised. The revision was made to include an additional circumstance under which a carrier may not require applicants to take the SHQ.
The new form is effective for coverage beginning on or after July 22, 2011.
We'll insert an updated exceptions page into the existing health questionnaire until we deplete our current print supply. We are also working to update the exceptions portion of our Individual enrollment application form and expect to have updated sales materials including the application available the week of July 25, 2011.
Work is also underway to update our online application process to reflect the revised exceptions. During our process to incorporate the updates to the SHQ, we may temporarily ask individuals who are applying online to print out their completed applications and mail them in.
