Updated 5208 requirements for groups coming from an Association
We recently updated our documentation requirements for groups coming from an Association, even if their current coverage is through us. We're now asking them to include their two most recently filed State of Washington Quarterly Tax Reports & Wage Detail Reports (Forms 5208 A and B), along with the equivalent for any out-of-state employees.
Terminated and new employees need to be indicated on the forms for the most recent quarter filed. Additionally, for the owner and any employees whose hours are not reported or whose hours are reported as zero (0) on Form 5208 B, we'll need a Verification of Employment (VOE) form plus three months of payroll records for the period prior to the effective date of coverage.
Our documentation requirements for micro groups are unchanged.
These changes will help us validate group- and member-specific eligibility and help protect the integrity of our community risk pool.
