Reminder: February deadline for new group submissions approaching
All required forms must be complete and submitted to us by the published deadlines to guarantee effective dates and rates; no exceptions. Incomplete or inaccurate information will delay the effective date of group coverage.
The following information and forms are required:
- Group Master Application
- Application for Enrollment/Change or Enrollment Spreadsheet
- Waiver Form
- Affidavit of Qualifying Domestic Partnership (if applicable)
- Affidavit of Qualifying Incapacitated Dependent Eligibility (if applicable)
For groups with March 1, 2011, effective dates, enrollment information will be due on the following dates:
- Feb. 22, 2011: Paper Enrollment Applications are due by 5 p.m. (Pacific time).
- Feb. 25, 2011: Online or Spreadsheet Enrollment Information is due by 5 p.m. (Pacific time).
To be consistent with these deadlines, Agent Center will close for quoting at 5 p.m. (Pacific time) on Feb. 22, 2011, for quotes for coverage effective March 1, 2011.
