04/29/2011   July 2011 GMAs now available

Group Master Application (GMA) forms (1 to 99 and 100+) for July 1, 2011 effective dates have been updated and will be posted to the Forms section of our Agent website, as well as to our Employer website and the Agent Toolkit. ASC GMAs are available through your Regence Sales contact.

Please note that our pre-filled GMAs available in Agent Center for renewing groups have not been updated to reflect these changes. We will notify you when those forms have been updated for your use.

Changes that have been made to these forms include new benefit options for Regence Innova®, mandated changes, updated verbiage and reformatting. To assist you with these changes, we created a Guide to Changes for each updated GMA.

As a reminder, groups renewing without any benefit plan, contribution or participation-level changes don't need to complete a GMA.
