Group Master Application (GMA) forms (1 to 99 and 100+) for Jan. 1, 2011, effective dates have been updated and posted to the Forms section of our Agent and Employer websites and the Agent Toolkits.

Numerous changes have been made to these forms, including the WA OIC-mandated 10-point font format, eligibility clarifications, additions for optional benefits and more. To assist you with these changes, we created a Guide to Changes for each updated GMA.

As a reminder, groups renewing without any benefit plan, contribution or participation-level changes don't need to complete a GMA.
