03/16/2010   Groups of 2-99 renewing 'as is' do not need to complete GMA

Groups of 2-99 that are renewing without any benefit or administrative changes can complete a Renewal Confirmation Form rather than a Group Master Application (GMA). A Renewal Confirmation Form is included with the renewal documents, right after the Underwriting Assumptions section.

This policy also applies to groups renewing on our ExpressionsSM and EncoreSM dental plans.

Any benefit or administrative changes (e.g., contribution levels, participation levels, etc.) or changes in group size that impact the application of COBRA, OBRA, TEFRA or DEFRA will require a GMA.
