The pre-printed GMA forms, used for groups 100+ on new products and for groups renewing on existing plans, will not change in 2010.

For groups of all sizes on Innova®, Engage®, ActivateSM and our Regence HSA Healthplan 2.0SM, the pre-filled and editable (fillable) GMA forms are available on Agent Center. To learn more about our GMA guidelines read our May 7 article.
