New SHQ available July 26
The Standard Health Questionnaire (SHQ) for Individual coverage
will change effective July 26 as additional exceptions have been
added. All applications submitted beginning July 26 are required
to use this new form. Additional exceptions have been added to the
health questionnaire regarding applicants eligible for COBRA and
applicants on non-COBRA eligible groups. New forms will be
available for online ordering shortly. Attached is a copy of the
new form.
