Individual updates: revised application and SHQ
New Application Deadline
Good news! Applications that are received by 5 p.m. on the last business day of the month will now be eligible for an effective date of the first of the following month when approved. This administrative change is effective immediately.
Applicants may still request a later effective date by following current administrative guidelines.
Washington State Health Questionnaire
Additional exceptions were recently added to the health questionnaire regarding applicants eligible for COBRA and applicants whose former employers are not COBRA-eligible. The new SHQ forms must be used starting July 26. As a result, our Application for Individual Coverage will also change effective July 26.
Currently there are three versions of the SHQ available. Please review the following to determine which version to use and where to get it:
- Standard Health Questionnaire dated April 1, 2008, for use prior to July 26, 2009
- Standard Health Questionnaire dated April 1, 2008, for use as of July 26, 2009. This form will be available on our Web site July 22.
- Determination of which of the above versions to use is based on postmark date, the date the form is received at our office or the date it is submitted online.
- Example: If an applicant is mailing their application on July 25 and it will be postmarked that day, then they would use the Standard Health Questionnaire for use prior to July 26. If they mail it on July 26 or after, they would use the Standard Health Questionnaire for use as of July 26.
- Standard Health Questionnaire for use with coverage that begins on or after Oct. 1, 2009. The form will be available on our Web site by Aug. 15.
- Use this Standard Health Questionnaire for any applications requesting an effective date of Oct. 1, 2009, or after.
All three versions are currently posted to the WSHIP Web site.
Individual Application
All Individual applicants who complete the SHQ as part of their application starting July 26, 2009, will need to use our new Application for Individual Coverage. This new document has two revisions:
- The exemptions to the Washington State Health Questionnaire have been updated.
- The application now includes a portability warning statement requested by the Office of the Insurance Commissioner.
