Group Master Applications and Benefits Selection Inserts have been updated
The 7/08 version of the Group Master Applications and Benefit Selection Inserts for groups of 2 to 50 and groups of 51+ are now available for your use for our FourFrontSM, Preferred, Selections®, Traditional, and Regence HSA Healthplans. When selling or renewing a group with an InnovaSM or EngageSM plan, please continue to use the Group Master Application dated 1/08. It is important that you use the appropriate forms to coincide with the group's effective date. To help you select the appropriate forms, we've restructured the Forms section of our Web site for easy reference.
