Online SHQ now available
Last February, the WSHIP Board revised the Standard Health Questionnaire. The changes were significant and, due to the extent of work required to update it, we removed this online form from our Web site in February.
We are pleased to announce that the updated online SHQ is now available, much earlier than we had originally anticipated!
