Individual HSA-Qualified Preferred Catastrophic plan members to receive corrected information
We recently sent each of our Individual plan members a packet containing a new contract and a letter describing the changes to their plan effective Jan. 1, 2008.
For our members on the Individual HSA-Qualified Preferred Catastrophic plan, we discovered that the contract contained incorrect information regarding the preventive care benefit. That information incorrectly stated that routine mammograms and prostate cancer screening services are not subject to the deductible. These services are subject to the deductible. To correct the error, we are mailing to impacted members a letter and a rider to modify their contract accordingly.
We apologize for any confusion or inconvenience the error caused. If your client received the contract rider, please remind them to keep the rider for their records.
