Washington State Standard Health Questionnaire revised
The Washington State Standard Health Questionnaire (SHQ) was revised effective July 22. The two main changes to the SHQ include revisions to Part I. A Exceptions:
- Exception 7, regarding losing coverage from a group that is exempt from COBRA, now includes church plans.
- Exception 8, a new rule with this revision, now includes Basic Health Plan disenrollment with the condition of 24-month continuous coverage and a 90-day window.
Since the first week of July, we have included the two-page WSHIP inserts in all Regence Individual sales packets. A supply of the inserts has also been provided to the Sales Executive team, Individual Marketing team, Membership team and all front-desk locations in Washington. However, the complete SHQ dated 07/22/2007 can be found on our website. Please begin to use this immediately.
